Session on Neurobiology of Epilepsy

Epilepsy is a chronic disorder with diverse clinical characteristics that preclude a singular mechanism a neurological disorder marked by sudden recurrent episodes of seizures due to disturbances in electrical signalling and its transmission in the brain. The fact that multiple mechanisms exist is not surprising given the varied ways the normal nervous system controls this balance. In contrast, understanding seizures in the brain of an individual with epilepsy is more difficult because seizures are typically superimposed on an altered nervous system.

Session on SUDEP: Sudden unexplained death in epilepsy

Sudden unexplained death in epilepsy (SUDEP) accounts for 7–17% of deaths among people with epilepsy. Patients with uncontrolled seizures are at greatest risk for SUDEP, and experiencing tonic-clonic seizures, taking multiple antiepileptic drugs and having coexisting neurologic disease further increase the risk.

Session on Metacognitive beliefs in Epilepsy

Emotional distress is common in people with epilepsy (PWE) for which efficacious interventions are required. Developing evidence-based treatments should be based on testable models of the psychological mechanisms maintaining psychopathology. The Self-Regulatory Executive Function (S-REF) model proposes that maladaptive metacognitive beliefs and processes are central to the development and maintenance of emotional distress. Although preliminary support exists for the role of metacognitive beliefs in emotional distress in PWE, their role has yet to be tested when controlling for the contribution made by illness perceptions

Majorly epilepsy is treated with antiepileptic drugs primarily first line drugs like carbamazepine, valproate, phenytoin, carbazepine whereas, second generation include zonisamide and lacosamide are showing good efficacy with low adverse effects. Ethosuximide is majorly used for primary treatment. Diazepam is now available as rectal gel so that can be applied by family members for prolonged seizures. Drug combination sometimes lead to adverse effects like fatigue and loss of appetite so, combinations are often replaced by monotherapy unless monotherapy achieves desired results. When medication doesn’t achieve the required targets doctor can sometimes suggest for surgery 64% cases showed seizure free cases who undergone surgery

Epilepsy is a chronic disorder usually occurred in children and people aged above 60 years. A neurological disorder marked by sudden recurrent episodes of seizures with loss of consciousness. It is associated with abnormal electrical activity in the brain with the loss of consciousness; it may be of different types and with different episodes of seizures from person to person. Epilepsy is a non-communicable disease with approximately 50 million people in the world suffers from this disease. People with Epilepsy respond to treatment approximately within 70% of the time. About three-fourths of people with Epilepsy living in low- and middle- income countries do not get the treatment properly as they need. In many parts of the world, people with Epilepsy and their families suffer from stigma and discrimination. Epilepsy can affect every one of us In Europe, at least 6 million people have epilepsy, and 15 million Europeans will have one seizure at some time in their lives. Nevertheless, in some countries of Europe, epilepsy is not recognized as a brain disorder and up to 40% of people with this condition may be untreated - the treatment gap.

Antiepileptic drugs are not always available or are not affordable and diagnostic facilities are lacking or are inadequate.
